Alzheimer’s, which accounts for roughly 70% of global dementia cases, is a leading cause of death among older adults. As we mark World Alzheimer’s Day, here’s a look at the latest progress in preventing, diagnosing, and treating this challenging condition.
How Effective Are the New Drugs?
After decades of setbacks despite billions invested in research, two drugs—Eli Lilly’s donanemab and Biogen and Eisai’s lecanemab—mark a turning point as the first treatments shown to slow Alzheimer’s progression significantly. However, their impact is modest, working only for early-stage patients, and they come with high costs and serious risks, including potentially fatal brain hemorrhages. This has fueled debate about whether their benefits justify the risks.
Lecanemab, sold as Leqembi, has gained approval in countries like the United States but faced pushback elsewhere. French health authorities have advised against reimbursing it, and the UK’s health service deemed both drugs insufficiently beneficial relative to their cost, highlighting the ongoing global divide in assessing these treatments.

Advances in Early Diagnosis
Diagnosing Alzheimer’s has traditionally relied on invasive, costly lumbar punctures, which can deter at-risk patients. A new blood test detecting disease biomarkers offers a simpler alternative and has been authorized in the US since May. Europe, however, has yet to approve such tests, with a UK trial recently launched to evaluate one. The key question is whether a blood test alone can reliably diagnose Alzheimer’s.
In the US, the Alzheimer’s Association now accepts biomarkers alone as sufficient for diagnosis. In contrast, European experts argue that a comprehensive clinical exam is still necessary to confirm cognitive and functional decline, as many with abnormal biomarkers never develop dementia, according to Dutch neurologist Edo Richard. Early diagnosis is critical, as some believe it could enhance the effectiveness of treatments like donanemab and lecanemab.
Can Alzheimer’s Be Prevented?
There’s broad agreement on risk factors for Alzheimer’s and dementia, with nearly half of cases linked to lifestyle factors like obesity, smoking, alcohol use, physical inactivity, and hearing loss, per a recent Lancet review.
Research is growing on whether lifestyle interventions—such as exercise and healthier diets—can reduce risk, but results are mixed. Randomized trials targeting these factors have shown little to no impact on cognitive decline, Richard notes. However, a recent JAMA study found slight improvements in cognitive decline after two years of intensive lifestyle support for Alzheimer’s patients.
For patients and families, progress may feel slow, but French epidemiologist Cecilia Samieri emphasized at a recent conference that these steps are significant compared to just a few years ago. She believes long-term trials, spanning 10 to 15 years, are needed to fully assess the impact of lifestyle interventions on a disease that develops over decades.
