Medical cannabis is more popular than ever for chronic pain, anxiety, insomnia and other common health concerns. But a major new analysis led by UCLA Health reveals a striking reality: public confidence in medical cannabis far surpasses the scientific evidence currently available.

A sweeping study published in JAMA reviewed more than 2,500 scientific papers published between January 2010 and September 2025 — including randomized clinical trials, clinical guidelines, and meta-analyses. Over 120 high-quality studies were prioritized based on sample size, relevance and condition type.

A 2018 U.S.–Canada survey shows just how widespread cannabis use has become: 27% of adults reported using cannabis or CBD for pain, anxiety, or sleep, often assuming strong scientific support. But the UCLA-led research suggests that assumption may not hold up.

Public Belief Is Outrunning the Science

Lead author Dr. Michael Hsu of UCLA Health explains that many people assume medical cannabis provides broad, proven benefits — even though the evidence remains limited for most conditions.

He emphasizes the need for clear clinician guidance to help patients make safe, evidence-based decisions.

Where Medical Cannabis Does Have Proven Benefits

The review notes that only a handful of FDA-approved, pharmaceutical-grade cannabinoid medications have demonstrated strong clinical benefits — and only for specific conditions, such as:

  • HIV/AIDS-related appetite loss
  • Chemotherapy-induced nausea and vomiting
  • Severe pediatric seizure disorders (including Dravet syndrome and Lennox-Gastaut syndrome)

Outside of these cases, evidence for medical cannabis is inconsistent, incomplete, or too limited to make firm recommendations.

Even though more than half of medical cannabis users take it for chronic pain, major clinical guidelines still do not recommend cannabis as a first-line treatment.

Potential Health Risks: What the Evidence Shows

The analysis also reviewed safety data and found notable concerns, especially with daily use or high-potency products.

Mental Health Risks

Long-term adolescent studies indicate potential correlations between high-potency cannabis and:

  • Psychotic symptoms (12.4% vs. 7.1% with low-potency use)
  • Generalized anxiety disorder (19.1% vs. 11.6%)

Addiction and Dependence

Around 29% of medical cannabis users met criteria for cannabis use disorder, underscoring the importance of monitored use.

Cardiovascular Concerns

Daily cannabis use — especially inhaled products — may increase the risk of:

  • Coronary heart disease
  • Heart attack
  • Stroke

Researchers caution that these risks may be under-recognized by patients and clinicians alike.

What Clinicians Should Consider

The review recommends that healthcare providers:

  • Screen patients for cardiovascular disease or psychotic disorders
  • Consider drug interactions
  • Evaluate whether potential harms outweigh possible benefits
  • Use caution when recommending THC-containing products

Study Limitations and the Need for Stronger Research

The authors note several limitations, including:

  • It was not a formal systematic review
  • No risk of bias assessment was performed
  • Some included research was observational, making it vulnerable to confounding
  • Clinical trial findings may not apply to all patients due to differences in product type, dose, and participant characteristics

Even so, the findings highlight an urgent need for more rigorous, standardized clinical trials to clarify both benefits and risks.
