Butter has long been one of life’s simple pleasures — yet for decades, it has carried a lingering sense of guilt. Like cheese and other full-fat dairy products, butter has been blamed for raising cholesterol levels and increasing the risk of heart disease due to its saturated fat content.
However, growing evidence suggests that for many people, cutting back on saturated fats may not significantly reduce the risk of death or cardiovascular disease — and for some, it may offer little benefit at all.

Saturated Fat’s Longstanding Reputation
Saturated fats, found naturally in foods such as red meat, butter, cheese, and yogurt, have historically been labeled dietary villains. Public health campaigns have linked them to clogged arteries, heart attacks, and strokes, making fat reduction a cornerstone of nutritional advice worldwide.
In the UK, the NHS currently recommends limiting saturated fat intake to no more than 30 grams per day for men and 20 grams for women, emphasizing that lower consumption is generally better for heart health.

New Research Challenges One-Size-Fits-All Advice
A major meta-analysis published in Annals of Internal Medicine is now challenging the long-held assumption that reducing saturated fat benefits everyone equally.
The analysis reviewed 17 studies involving more than 66,000 participants and found that only people at high cardiovascular risk showed meaningful health benefits from cutting back on saturated fats. For individuals at low cardiovascular risk, the impact was minimal.
“For persons at low cardiovascular risk, reducing or modifying saturated fat intake has little or no benefit over a period of five years,” the researchers concluded.
Among high-risk individuals, however, the evidence showed reductions in mortality and major cardiovascular events, suggesting dietary changes may still be crucial for certain populations.

Rethinking the Diet–Heart Hypothesis
In an editorial published alongside the study, Dr. Ramon Estruch and Dr. Rosa Lamuela-Raventós of the University of Barcelona argued that scientific understanding of saturated fats is evolving.
Quoting Bob Dylan’s The Times They Are A-Changin’, they wrote that saturated fats are no longer viewed as universally harmful. Instead, some types of saturated fatty acids may even offer protective benefits when consumed as part of a balanced diet.
The researchers questioned the long-standing “diet-heart hypothesis,” which emerged in the mid-20th century and linked saturated fats to heart disease primarily through their effect on cholesterol levels. According to the authors, much of that theory was based on associations rather than direct causation.
They noted that diets rich in unsaturated fats — alongside certain short-, medium-, and odd-chain saturated fats — may support cardiovascular health when overall diet quality is high.
Experts Urge Caution
Despite the findings, experts warn against making sweeping changes to dietary guidelines just yet.
Professor Nita Forouhi, a population health and nutrition specialist at the University of Cambridge who was not involved in the research, pointed out that the study did not examine long-term outcomes over the 10-year timeframe typically used to assess heart disease risk.
“For this reason, it would be premature to change existing dietary guidelines recommending saturated fat intake below 10 percent of total energy intake,” she said.

What This Means for Everyday Diets
The takeaway is not that butter and cheese should be eaten without limits — but rather that blanket advice to eliminate saturated fats may not be appropriate for everyone.
For people without significant cardiovascular risk factors, enjoying moderate amounts of full-fat dairy as part of a balanced diet may not pose the danger once assumed. Diet quality, overall lifestyle, and individual health risk appear to matter far more than singling out one nutrient.
